Influenza

IM

·          Age ≥50 yrs.

·          Age 6mo-50yr with heart, kidney, lung diseases, diabetes, hemoglobinopathies, immunosuppression,, health care workers. 

·          Egg allergy.

·          Previous anaphylactic reaction.

·          Acute illness.

Given every year (preferably October thru November).

 

Pneumococcal 

IM / SC

·          Age ≥65 yrs.

·          Age 2-65yrs chronic cardiac , renal, pulmonary, liver diseases, alcoholism, diabetes mellitus, CSF leaks, asplenia, sickle cell disease, malignancies, lymphomas, leukemias.

·          Previous anaphylactic reaction.

·          Acute illness.

One-time dose.

One-time booster 5yrs later for people with chronic diseases or to those who received their 1st dose before age 65.

Hepatitis-B

IM

  • All adolescents.
  • Close contacts & sex-partners of HBsAg +, injectable drug users, male homosexuals, patients with STDs, health care workers, patients on hemodialysis, anti-HCV +.
  • Previous anaphylactic reaction.
  • Acute illness.

3 doses at 0, 1 & 6 months.

(Alternatively at 0, 1 & 4 or 0, 2 & 4 months)

Hepatitis-A

IM

·          Chronic liver disease, all anti-HCV +, HBsAg + with chronic liver disease, drug users, male homosexuals, bleeding disorders, food handlers.

·          Travelers (except to Canada, Japan, Western Europe, Australia & New Zealand)

  • Previous anaphylactic reaction.
  • Acute illness

·          Pregnancy (but can be given to breast-feeding mothers).

2 doses at 0 & 6 months.

Measles, Mumps, Rubella (MMR)

SC

·          Women of child-bearing age

·          Health care workers, students entering colleges, international travelers.

·          Adults born in or after 1957 without serologic proof of immunity.

  • Previous anaphylactic reaction.
  • Pregnant women or possibility of pregnancy within 4 wks.
  • Immunocompromised due to malignancies, steroids, radiation.
  • Recipients of blood, plasma or Ig within past 11 mo.
  • Acute illness.

One or two doses 4 wks apart (two doses for high risk individuals).

Tetanus, Diphtheria (Td)

IM

·          Adolesecnts & adults

·           

  • Previous anaphylactic reaction.

·          Acute illness

·          For  unvaccinated, give 3 doses at 0, 1-2mo & 6-12mo.

·          For vaccinated, give a booster every 10yrs.

·           

Varicella  (Chickenpox)

SC

·          Health care workers, teachers of young children, adolescents and adults living in households with children, military personnel, close contacts of immunocompromised, workers in day care institutions, residents and staff members in institutional settings.

·          Women of child-bearing age.

·          International  travelers.

  • Previous anaphylactic reaction.
  • Acute illness
  • Pregnant women or possibility of pregnancy within 4 wks.
  • Immunocompromised due to malignancies or AIDS.
  • Recipients of blood, plasma or Ig within past 11 mo.
  • Aviod salicylates for 6wks after vaccination for risk of Reye’s syndrome.

·          Two doses 4-8wks apart.

·          Give Varicella & MMR vaccine atleast 4wks apart.

Poliomyelitis

IM or SC

Meningococcal

SC

·          International travelers to sub-Saharan Africa & to Saudi Arabia for Hajj.

·          Anatomic or functional asplenia.

·          Terminal complement component deficiency.

·
